Chronic Care Management (CCM) is a Medicare-supported program that provides structured, ongoing support for patients living with two or more chronic conditions — such as diabetes, hypertension, heart disease, COPD, thyroid disease, or kidney disease. Personal Physician Care offers a fully implemented CCM program at all three South Florida locations.
Enrolled patients receive: a comprehensive, personalized care plan updated at least annually, at least 20 minutes of non-face-to-face care coordination per month from a dedicated care manager, 24/7 access to a care team member for urgent non-emergency needs, medication reconciliation and management support, coordination with specialists (including PPC's in-house endocrinologists and pulmonologist), support for scheduling and follow-through on preventive services, and regular review of lab results with proactive outreach when values are out of range.
Patients with multiple chronic conditions are at significantly higher risk of emergency department visits and hospital admissions — often due to medication errors, missed follow-ups, or early symptoms going unaddressed. PPC's CCM program creates a structured safety net: someone is actively monitoring your care between visits, not just waiting for you to call. Studies show that patients enrolled in CCM programs have fewer ER visits, better medication adherence, and improved disease control.
For Medicare beneficiaries, Chronic Care Management is a covered benefit — typically with little to no out-of-pocket cost. Ask your PPC provider whether you qualify. Most patients with two or more diagnosed chronic conditions are eligible.
